What to Expect
In an effort to keep dental costs down while maintaining a high level of professional care, we have established the following payment plans for the use of our patients. Treatment may be paid as follows:
- Prepayment of payment at the time of treatment with a courtesy off.
- Payment of balance in full within 10 days of statement date.
- Use of your Visa, Mastercard, Discover or American Express
Extended payments are available upon special request. Accounts outstanding more than 60 days from treatment date will bear interest at a rate of 1.5% per month (or 18% annum.)
All major treatment and treatment involving a laboratory procedure will require an appropriate down payment.
We also offer payment plans through Care Credit. We offer up to 12 months no interest or extended (2-4 year) payment plans through Care Credit! To apply for online payment, follow this link www.carecredit.com
Patients are seen by appointment only. Please call in advance so that we may reserve a time for you. The office telephone number is (630) 545-0151. We make every effort to be on time for our patients, and we ask that you extend the same courtesy to us. If you cannot keep an appointment, please notify us immediately. If possible, please give us 48 hours notice. This courtesy on your part make it possible for us to give your appointment to another patient.
If you have insurance, we will help you to estimate the coverage you have available. We ask that you assign you insurance benefits to us. The balance will be arranged for you to pay as listed above. Professional care is provided to you, our patient, and not to an insurance company. Thus, the insurance company is responsible to the patient and the patient is responsible to the doctor. We will help in every way we can by filing your claim and handling insurance questions from our office on your behalf.
We reserve a limited amount of time each day so that the emergency patients may be scheduled quickly. If you have a problem requiring emergency treatment, please call as early in the day as possible.
Should you have an emergency after hours or on weekends, the doctors can be reached at their homes; the telephone numbers are listed in the local directory.
Recare and Hygiene
We believe in the importance of regular preventative dental care. As a service to our patients, we have adapted a customized recare program suited to each individual’s needs. We stand behind the quality of the dentistry that we provide our patients. We guarantee our crowns and CEREC restorations for 5 years from cracks, chipping or any breakage; as long as the patient stays current on their hygiene visits twice a year (once every six months.
